Eligibility Requirements

• Justification of Emergency (written proof; letter from client)
• Proof of all Household Income (all documents must be current)
• Must bring rent receipts (last three)
• Social Security Cards of all Household Members
• Insurance Card: Current Medicaid Card/HMO or Health Insurance Card
• Photo Identification: Driver's License, County Welfare or other
• Utility Assistance: Must bring current utility bill
• Rental: Must bring court order or lock out notice
• Shelter: Proof of Homelessness
